Collection of Information
We require visitors and/or customers who wish to apply for insurance and/or purchase
the products and/or services offered on our Site to initially give us the following
information: the state in which they currently reside, gender, date of birth, and
e-mail address. Subsequently, we may request some or all of the following additional
information: company name, address, phone number, and e-mail address, basic medical/lifestyle
information, such as, but not limited to, family disease history, smoking history,
health complaints, present medical/health condition, financial qualification information,
and billing information (including billing name and address, and credit card number).
Other information relevant to an insurance application, such as beneficiary information,
family information, and the like, may also be collected.
We use the information we collect to identify products in which you have expressed
an interest, and to determine whether you qualify for these products. We may forward
this information on to underwriters, qualified insurance agents, and/or insurance
carriers for them to evaluate and/or process in order to determine whether you qualify
